If your Alumni Association (AA) has joined the Unified Application System, just download and print out the Online Application Form for completion.  

If your AA has not joined the Unified Application System, please contact the Federation Office for a copy of application form for your AA.

All members of the alumni associations  (AAs) under the Federation are eligible to apply for the "Federation card" issued by the Federation of the PolyU Alumni Associations.
